Carrollton Volunteer Fire Department was dispatched at 5:54 p.m. 28 April 2014 to a reported "head-on collision" at the north end of the Hazelwood (Crittenden) Bridge over Chuckatuck Creek into Suffolk. Carrollton VFD units Squad-10, Rescue-10 and Medic-11 responded to the scene and upon arrival reported "heavy damage...working entrapment." Carrollton VFD Assistant Chief G. Rawls assumed incident command and volunteer crews began immediate extrication operations. Hand hose lines were deployed as a precaution in the event of fire and both vehicles were mechanically stabilized. City of Suffolk law enforcement was requested to assist with traffic control on the opposite end of the bridge due to heavy rush hour volume of traffic. Virginia State Police unit arrived to assist with traffic at the Isle of Wight County end of the bridge and to begin investigation and reporting protocols. The entrapped driver was extricated using hydraulic cutting tools and then secured on a stretcher for movement to ambulance unit Squad-10. The EMS crew medically stabilized the patient and transported to the nearest emergency trauma center. R-10 and M-11 crews remained on scene until dispatched at 6:22 p.m. to a second motor vehicle collision at the James River Bridge.
